We’ve combined health coaching rooted in behavioural psychology with digital education, delivering outcomes that stand up to rigorous evaluation.

8:20: Case Study of the Year Entry: Birmingham and Solihull Mental Health NHS Foundation Trust: The MERIT programme has developed an Electronic Health Record Viewer which provides access to patients’ mental health histories to clinicians in four NHS trusts in the West Midlands, and a Bed Finder to make more efficient use of inpatient beds and reduce the incidence of long-distance out-of-area placements.

8:19: Case Study of the Year Entry: NHS North of England Commissioning Support Unit: The Value Based Commissioning programme identified the opportunity to reduce clinical risk for patients through implementing a region-wide approach to managing procedures of limited clinical value. Since August 2017, the web-based system VBC Checker has assisted in a reduction of 15,000 procedures, saving £24m across the Cumbria and North East health economy.

The aim is to encourage service users to engage in their therapeutic work by implementing a virtual world to encourage narrative from users. The technology can also be applied to educational workshops and reflective practices.

8:14: Efficiency Savings of the Year Entry: Babble ltd:  Babblevoice, the phone system for primary care, better supports practice patients and staff. Babblevoice improves practice efficiency by: Automate appointment booking without speaking to reception, when patients call automatically retrieve and display patient records and capture patient’s new phone numbers and store for future use.

8:14: Efficiency Savings of the Year Entry: Taunton & Somerset NHS Foundation Trust with Ingenica Solutions:  Taunton and Somerset NHS Foundation Trust implemented Ingenica Solutions’ 360 inventory management solution to manage stock, and achieve patient level costing.  A benchmark for success, with outstanding results, the project has improved productivity, operational efficiency, achieved cost savings, increased stock control and visibility, enabled product standardisation and rationalisation and reduced wastage.

As a result of this work there have been significant improvements in many quality indicators at a time when staffing numbers have continued to fall as demand has continued to rise.

7:52: Staff and Patient Experience Entry: Cambridgeshire Community Services NHS Trust:  Cambridge Community Service NHS Trust’s GP liaison service – believed to be the only nurses-led service of its kind in the country – streamlines the way GPs refer patients for same day hospital assessment in Luton. The team provides a fast, effective patient pathway through the hospital and back into the community.

7:52: Staff and Patient Experience Entry: Evergreen Life:  Evergreen Life is a personal health record app that stores your health information in one place. Connected to all three GP systems in England, it also enables people to book appointments, order repeat prescriptions and view their medical record online and all from the palm of your hand.

7:50: Staff and Patient Experience Entry: NHS Norfolk Children and Young People’s Health Services:  JustOneNorfolk.nhs.uk is a new, innovative NHS digital platform which aims to help children, young people and their families in Norfolk to have a better understanding of their health, wellbeing and care. The website launched in November 2018 and has already had over 46,000 website views.

7:36: Patient Safety Entry: Yorkshire & Humber Academic Health Science Network:  Yorkshire Safety Huddles – delivered by the Yorkshire and Humber Patient Safety Collaborative (PSC) – enable multi-disciplinary teams to work together to reduce the risk of harm to hospitalised patients. More than 6,600 falls have potentially been avoided – equating to around £17 million saved in healthcare costs based on cost estimates from NHS Improvement.

7:35: Patient Safety Entry: Warrington and Halton Hospitals NHS Foundation Trust:  In order to strengthen a “board-to-ward“ culture of patient safety, a Trust Wide Executive-led Safety Huddle was introduced in May 2018; the purpose is to ensure that any safety concerns are addressed as soon as possible and lessons learnt are shared across the organisation. Senior leaders from across the organisation must be in attendance for this to be effective and it is essential that staff who are able to influence practice and processes within the Trust are core members, in addition to representation from both clinical and non-clinical colleagues.

7:35: Patient Safety Entry: Birmingham and Solihull Mental Health NHS Trust:  The Physical Observation “App” is the second phase of the Digital Ward project. The “app” enables clinicians to record physical observations at the point of care. The “App” also enables a service user’s plan and observation history to be detailed in a place which is accessible to all.

7:33: Patient Safety Entry: Deontics: Revolutionary AI Patient Decision Aid reduces unnecessary risk to patients whilst potentially saving £77M per year for colorectal cancer patients. Deontics in conjunction with the Royal Free Hospital developed a decision aid for patients with stage II colorectal cancer considering whether or not to have adjuvant chemotherapy. The AI tool enables individualised, evidence based clinical decisions to be jointly made. 92% rejected unnecessary chemotherapy saving potentially £77M p.a. nationally.

Seniors are empowered to stay in their own homes, knowing that 24-hour support is on hand. The company is CQC registered and its algorithm finds the best carer based on customer needs and interests.

7:25: Improving Outcomes Entry: Derbyshire Community Health Services NHS Foundation Trust:  Improving outcomes for patients with leg ulceration. Programme designed and aimed at all Band 6 District Nursing Sisters and Community Nursing Sisters to improve leadership skills with a focus on quality conversations (health coaching), improved education on assessment and treatment options, Leadership and Tissue Viability Mentorship.

7:24: Improving Outcomes Entry: Cambridgeshire Community Services NHS Trust:  Cambridge Community Service NHS Trust’s GP liaison service – believed to be the only nurses-led service of its kind in the country – streamlines the way GPs refer patients for same day hospital assessment in Luton. The team provides a fast, effective patient pathway through the hospital and back into the community.

7:23: Improving Outcomes Entry: Birmingham and Solihull Mental Health NHS Foundation Trust:  Reach Out is an innovative partnership of NHS Trusts and a private provider who are concluding a 2 year pilot within Adult secure care, implementing a new care model which will reduce the amount of patients who are placed outside of the West Midlands which impacts on their recovery journey.

7:23: Improving Outcomes Entry: NHS North of England Commissioning Support:  Built in partnership with NHS England North, local authorities and care homes, the web-based Capacity Tracker instantly connects users with care homes across England that have vacancies. This has been shown to help optimise patient flow, reduce avoidable delayed transfers of care and therefore length of stay in inpatient settings.

Zenker’s diverticulum (ZD) is a rare condition which tends to affect elderly patients with significant co-morbidity, where an outpouching is formed in the lower part of the throat. ZD significantly affects people’s quality of life. #leadinghealthcareawards
